You can update yourself:
- Phone number — in your account profile settings
- Address — in your account profile (note: addresses can only be updated once every 6 months)
Contact our Patient Support Team to update:
- Email address — handled by our team for security reasons; we may ask you to verify your identity
- Legal name — requires supporting documentation (marriage certificate, court order, or updated government-issued ID)
- Address — if you need to update it within the 6-month window
