- A note excusing you from work or school
- A return-to-work or return-to-school health clearance note
We are not able to write:
- Disability forms
- Workers' compensation forms
- Vaccine or mask exemptions
- FMLA paperwork
Your note will be emailed as a PDF shortly after your visit. During busy periods, delivery may take up to 12 hours.
