How do I upload my medical history or patient records?

1. Log into your account at doctorvisit.quick.md
2. Go to your Profile
3. Navigate to the Files or Documents section
4. Upload your records (accepted formats: PDF, JPEG, PNG)

Useful documents to upload:

• Previous lab results (FSH, estradiol, TSH, etc. for MHT)
• Prior prescriptions
• Relevant medical history

After uploading, let your clinician know at the start of your appointment so they can locate your documents.
