Yes, but the program is limited. QuickMD’s financial assistance program is only for new patients starting our Addiction Treatment service who are having trouble covering the cost of their second visit. It does not apply to your first visit, any visit after your second, or any other QuickMD service.
To apply, you’ll need to complete our Financial Assistance Form and provide evidence of financial need, such as an unemployment letter or proof of income. Once you submit your form, we’ll get back to you within 48 hours.
Who qualifies
You’re eligible if you are new to QuickMD and enrolled in Addiction Treatment. Existing QuickMD patients can’t apply, and the program doesn’t cover other types of care. [Add any other exclusions here, e.g., medications, labs, insurance interactions.]
How to apply
Complete the Financial Assistance Form here: https://qmdfinancialassistance.zendesk.com/hc/en-us/requests/new
Be sure to include documentation of your financial need when you submit it. You can fill out the form on your own, but if you have any trouble, contact Support and we’ll help.
Getting the form from Support
If you reach out to QuickMD Support, we’ll send you the form in whatever way you contacted us. In chat, we’ll share the link right in the conversation. By email, we’ll reply with the form. If you call, we’ll ask whether you’d like it by email or text. We can text it to the number you’re calling from if it matches the number on your account. Otherwise, we’ll email it to you.
