The intake form can only be accessed prior to each visit. To fill it out, please follow these steps:
Log in to your account.
Click on 'Upcoming Visits'
Locate your appointment and find the 'Intake Form' section.
Click on the form and complete the requested information.
Submit the completed form.
If you're still having trouble accessing the form, make sure your appointment is scheduled and check the timing to ensure the form is available before your visit.
